Colleen O'Leary, MSN, AOCNS, from James Cancer Hospital, at the Ohio State University Medical Center describes chemotherapy safety standards.

O'Leary says that various guides, organizations and advisory panels advocate safety standards that include an independent double check for all administration of chemotherapy which includes checking the right dose, the right medication, the right patient, calculating the BSA, and verifying height and weight. Having all that done independently and then documenting that each step was done helps to ensure the safest practice in giving the right medication to the right patient without under-dosing or overdosing the patient.
